PACTo Care and Treatment Access Program

PACTo 2A collaborative project that seeks to create healthier communities and help persons over 18 years of age gain access to clinical services. PACTo aims to evaluate the effectiveness of an intervention model targeted at persons whose particular circumstances could restrict access to care and treatment services.

The program helps participants navigate the system and the required procedures so they can receive care and treatment services. It also helps participants access services by bringing medical care services to locations at needed times.  The project’s evaluation cohort started in 2013 and its clinical intervention phase commenced in mid 2015.

PACTo currently provides services in the towns of San Juan, Vega Alta, Vega Baja, Manatí, Arecibo, Loíza, Carolina, Trujillo Alto, Gurabo, Caguas, Cataño, Bayamón and Guaynabo. Phase 1 aims to assist 400 persons and Phase 2 is projected to successfully connect 750 persons to care and treatment services.

The team comprises health care professionals, a driver, research assistants and patient navigators. PACTo is a collaborative project among Iniciativa Comunitaria, Columbia University and the University of Puerto Rico School of Medicine.
